Form I-90 Document Checklist

Most Form I-90 applications need only a few documents, but what you include depends on why you are filing. Use this checklist to gather the common items.

Who this guide is for

  • Permanent residents preparing Form I-90
  • Anyone replacing a lost, damaged, expiring, or incorrect green card

For most applicants

  • A copy of the front and back of your current or expired Permanent Resident Card, if you have it
  • If you do not have your card, a copy of a government-issued photo ID

Depending on your reason

  • Name change: a copy of the marriage certificate, divorce decree, or court order
  • DHS error on the card: the original card with the incorrect information (USCIS does not accept a copy in this case), plus proof of the correct information
  • Damaged card: a copy of the card or of a government-issued photo ID
  • Card never received: a copy of the latest Form I-797 notice for the application that should have produced the card, plus a copy of a government-issued photo ID

Important considerations

  • Send legible photocopies unless the instructions ask for an original. The main exception is a card with a DHS error, where the original card is required.
  • Bring your appointment notice and photo ID to any biometrics appointment.
  • Keep a copy of everything you send and your receipt notice.

When to consider legal help

Consider speaking with a qualified immigration attorney or accredited representative if your situation involves any of the following:

  • Any arrest, criminal charge, or conviction, in any country
  • Past or current removal (deportation) proceedings, or a prior removal order
  • Unlawful presence, a prior visa overstay, or a past immigration violation
  • Questions about whether you need a waiver or qualify for an exception
  • A card that was taken by an official or a notice questioning your status
